It's important to know what to do if you get hurt at work. Watch this video or visit our website to learn what to do.
REMEMBER: Culinary providers are NOT allowed to charge you to fill out LOT or Disability Forms. You may still have to pay a copay for your visit.
If you have been charged to fill out paperwork, save your receipt and call Customer Service at 702-733-9938.
New In-Network Radiology Providers
We have made changes to our radiology network:
- Starting April 1, Pueblo Medical Imaging will be part of the Culinary network!
- SimonMed Imaging is now part of the Culinary network! Walk-in x-rays are available at all locations.
- As of April 1, Desert Radiology will no longer be in the Culinary network.

This year, 13 students at UNLV received the John Wilhelm Endowed Scholarship, totaling $13,000 awarded.
This year's winners are pursuing degrees in public health, healthcare administration and more.
